Who Should Apply?
We encourage candidates from a variety of backgrounds with strong quantitative skills to apply. For admission eligibility to the PhD in Program in Health Services Research, applicants must have completed two semesters of calculus (preferably 3 semesters) and a semester of statistics.
When Can I Apply?
The PhD program in Health Services Research admits students for the fall semester every year. Potential candidates must have their application packet complete by January 15th to receive full consideration for admission as well as financial aid.
After May 15th, please contact the program directors (Rebecca_Thornton@Baylor.edu and Scott_Cunningham@baylor.edu) to see if applications are still being accepted.

Essays
Personal essays are an opportunity to express your unique qualifications, experiences and reasons for applying to the PhD program. Prepare your essays carefully, using details and specific examples.
Resume
A current resume is requested, specifically describing work experiences, activities, honors and awards.
Letters of Recommendation
You will need to provide letters of recommendation from two individuals who know you well and can present balanced evaluations and assessments. It is preferred that your recommendations will be from former professors.
Transcripts
Please provide official transcript(s) from every college you've attended.
Application Fee
A $50 non-refundable fee, payable by credit card or check, is required to process your application.
GMAT/GRE/TOEFL
You are required to take the GRE exam. There is no minimum GRE score requirement. NOTE: Unofficial copies of the test scores are acceptable for the evaluation process. For students who are not U.S. citizens, you are required to take the TOEFL test*.
*The exception is if you have an undergraduate or graduate degree from a US university.
International Applicant Requirements
English Language Tests
All foreign national and permanent resident applicants are required to provide a score from either the TOEFL, IELTS or PTE Academic.
Foreign Transcripts
If you are an international applicant, or attended a foreign university, please follow the guidelines for foreign transcripts available on the Graduate School website.
Acceptance to the Program
Upon admission to the Program, you will receive notification of acceptance from the Baylor Graduate School as well as the PhD in Health Services Research Program Director.
The Graduate School letter will outline conditions of acceptance, information on financial awards, as well as other general information pertaining to student housing and various student-related services.
The letter from the Program Director will provide Entrepreneurship program-specific details that will be important to you prior to the start of fall classes.
If you have prepared one or more empirically oriented research studies (that is, a research project involving data analysis), the admissions committee would be very interested in seeing your best work. Please submit up to two papers with your application.
The admissions committee may invite you to schedule an interview to ask questions about your application. The interview will be either in person or virtual (e.g., Skype) as circumstances warrant.
